Summary

Voclosporin, a calcineurin inhibitor, can cause nephrotoxicity, hypertension and hyperkalemia. Combined with valsartan the risks of hyperkalemia and reduced glomerular filtration increase, and diuretic-related volume depletion adds to renal risk.

What this interaction means in plain words

Hydrochlorothiazide-valsartan and Voclosporin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Hydrochlorothiazide-valsartan belongs to Cardiovascular; Voclosporin belongs to Miscellaneous.

Evidence level: Moderate

Why it happens

Calcineurin inhibitor afferent arteriolar vasoconstriction combined with ARB efferent dilation and reduced aldosterone effect impairs GFR and potassium excretion.

What to do

Monitor eGFR and serum potassium closely, especially after initiation and dose changes. Reduce or interrupt voclosporin according to labeling if eGFR falls or potassium rises.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
