Summary
Fluoroquinolones carry a risk of tendinitis and tendon rupture, and concurrent systemic corticosteroid use increases that risk further. The Achilles tendon is most often affected, and older patients are most vulnerable.
What this interaction means in plain words
Hydrocortisone and Moxifloxacin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Hydrocortisone belongs to Endocrine; Moxifloxacin belongs to Anti Bacterial.
Why it happens
Additive tendon toxicity; fluoroquinolones and corticosteroids both impair tenocyte function and collagen integrity.
What to do
Avoid the combination where an alternative antibiotic is suitable, especially in patients over 60 or with renal impairment. Stop the quinolone and rest the limb at the first sign of tendon pain.
