Summary
Tipranavir (usually given with ritonavir) is a net strong CYP3A4 inhibitor and so can increase hydrocortisone exposure. Hydrocortisone might weakly lower tipranavir levels, but this is less relevant.
What this interaction means in plain words
Hydrocortisone and Tipranavir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrocortisone belongs to Endocrine; Tipranavir belongs to Anti Virals.
Why it happens
Ritonavir-boosted tipranavir inhibits CYP3A4, increasing hydrocortisone exposure, while weak hydrocortisone induction may slightly reduce tipranavir.
What to do
Monitor for signs of glucocorticoid excess with prolonged use and reduce the hydrocortisone dose if needed. Follow HIV viral load as scheduled.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
