Summary

Ibuprofen can reduce the antihypertensive effect of irbesartan and, in volume-depleted, elderly or renally impaired patients, can precipitate acute kidney injury. Hyperkalemia may also occur.

What this interaction means in plain words

Ibuprofen and Irbesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Ibuprofen belongs to Musculoskeletal; Irbesartan belongs to Cardiovascular.

Evidence level: High

Why it happens

NSAIDs block prostaglandin-mediated afferent arteriolar dilation while ARBs dilate the efferent arteriole, lowering glomerular filtration; NSAIDs also cause sodium retention and reduce renin and aldosterone.

What to do

Monitor blood pressure, serum creatinine and potassium, and ensure adequate hydration; avoid regular NSAID use in high-risk patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
