Both agents prolong repolarization, and ibutilide already carries a substantial risk of torsades de pointes. Adding chlorpromazine increases the likelihood of life-threatening ventricular arrhythmia.
What this interaction means in plain words
Chlorpromazine and Ibutilide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Chlorpromazine belongs to Central Nervous System; Ibutilide belongs to Cardiovascular.
Why it happens
Additive QT prolongation from cardiac potassium channel blockade.
What to do
Avoid giving ibutilide to patients taking chlorpromazine; use an alternative cardioversion strategy or hold the phenothiazine as appropriate.
