Summary

Secobarbital may induce enzymes that convert ifosfamide to its active metabolites, raising active metabolite formation. This can increase toxicity, including neurotoxicity and myelosuppression, and also shift production toward neurotoxic chloroacetaldehyde.

What this interaction means in plain words

Ifosfamide and Secobarbital can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Ifosfamide belongs to Oncology; Secobarbital belongs to Central Nervous System.

Evidence level: Low

Why it happens

Induction of CYP2B6 and CYP3A4 by secobarbital increases ifosfamide bioactivation and metabolite formation.

What to do

Avoid enzyme-inducing barbiturates if possible. If they are used, monitor closely for CNS toxicity, myelosuppression and urotoxicity.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
