Summary

Iloperidone blocks alpha-1 adrenoceptors, which can cause orthostatic hypotension, and phenylephrine acts as an alpha-1 agonist. The antipsychotic may therefore blunt the pressor response to phenylephrine, so a larger dose may be needed to achieve the desired blood pressure.

What this interaction means in plain words

Iloperidone and Phenylephrine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Iloperidone belongs to Central Nervous System; Phenylephrine belongs to Respiratory.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism at alpha-1 adrenergic receptors by iloperidone opposing phenylephrine agonism.

What to do

Monitor blood pressure closely when phenylephrine is given for hypotension in a patient on iloperidone and titrate to effect. Avoid using epinephrine for this purpose since unopposed beta effects may worsen hypotension.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
