Summary
Iloperidone can inhibit CYP3A4 and may raise zolpidem exposure, which is partly cleared by this enzyme. Higher zolpidem levels can deepen sedation and increase next-day impairment and complex sleep behaviours. Iloperidone itself is sedating, so the effects may be additive.
What this interaction means in plain words
Iloperidone and Zolpidem can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Iloperidone belongs to Central Nervous System; Zolpidem belongs to Central Nervous System.
Why it happens
Iloperidone weakly inhibits CYP3A4-mediated zolpidem metabolism, with additive CNS depression.
What to do
Start with the lowest zolpidem dose and monitor for excessive sedation, dizziness and psychomotor impairment. Reduce the zolpidem dose if daytime drowsiness occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
