Summary

Mavacamten is metabolized mainly by CYP2C19 and CYP3A4, and imatinib inhibits CYP3A4 (and possibly CYP2C19). Higher mavacamten exposure may reduce ejection fraction and cause heart failure from systolic dysfunction.

What this interaction means in plain words

Imatinib and Mavacamten can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Imatinib belongs to Oncology; Mavacamten belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Imatinib inhibits CYP3A4 and may inhibit CYP2C19, decreasing mavacamten clearance.

What to do

Check LVEF by echocardiography and adjust the mavacamten dose, with closer monitoring when imatinib is started or stopped. Consider an alternative to imatinib if left ventricular function declines.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
