Summary

Tricyclic antidepressants can modify responses to sympathomimetics. Because isoproterenol is a direct beta agonist not dependent on neuronal uptake, the effect is less pronounced than with norepinephrine, but tachycardia and arrhythmias may be more likely in patients on a tricyclic, which has its own cardiac effects.

What this interaction means in plain words

Imipramine and Isoproterenol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Imipramine belongs to Central Nervous System; Isoproterenol belongs to Respiratory.

Evidence level: Low

Why it happens

Tricyclic-induced alterations in adrenergic neuronal reuptake and additive cardiac effects; direct beta agonism is largely unaffected by reuptake blockade.

What to do

Monitor heart rate, blood pressure and ECG when isoproterenol is given to a patient on imipramine, and titrate the isoproterenol dose cautiously.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
