Summary
Protriptyline is a tricyclic antidepressant that prolongs the QT interval and has strong anticholinergic effects. Together with indacaterol it may raise the risk of arrhythmia, and it adds to the antimuscarinic effects of glycopyrronium.
What this interaction means in plain words
Indacaterol-glycopyrronium and Protriptyline can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive QT prolongation, enhanced adrenergic cardiovascular effects, and additive antimuscarinic activity.
What to do
Monitor ECG, potassium and heart rhythm, and watch for anticholinergic effects such as urinary retention and constipation. Choose a less arrhythmogenic antidepressant if feasible.
