Summary
Indomethacin can reduce the blood pressure lowering effect of valsartan. In older, volume-depleted or renally impaired patients the combination can cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Indomethacin and Valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Indomethacin belongs to Musculoskeletal; Valsartan belongs to Cardiovascular.
Why it happens
NSAIDs block vasodilatory prostaglandins and reduce afferent arteriolar flow, while angiotensin receptor blockade dilates the efferent arteriole, lowering glomerular filtration pressure and impairing potassium excretion.
What to do
Monitor blood pressure, serum creatinine and potassium within the first weeks and after dose changes, and ensure adequate hydration. Avoid or limit NSAID use in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
