Summary

The intranasal influenza vaccine contains live attenuated virus. In a patient immunosuppressed by infliximab, the vaccine strain could replicate and cause disease, and the immune response may also be reduced.

What this interaction means in plain words

Infliximab and Influenza virus vaccine quadrivalent intranasal are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

TNF inhibition impairs immune control of live attenuated virus replication and reduces vaccine response.

What to do

Avoid the live intranasal vaccine during infliximab therapy; use an inactivated injectable influenza vaccine instead.
