Summary

Isoflurane can prolong the QT interval, and salbutamol can add to this through beta-agonist-related hypokalemia and increased heart rate. Halogenated anesthetics may also sensitize the myocardium to catecholamines, raising the risk of arrhythmia during anesthesia. The effect of inhaled salbutamol at normal doses is usually small.

What this interaction means in plain words

Ipratropium-salbutamol and Isoflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Ipratropium-salbutamol belongs to Respiratory; Isoflurane belongs to Central Nervous System.

Evidence level: Low

Why it happens

Additive QT prolongation and myocardial sensitization to beta-adrenergic stimulation, with possible salbutamol-induced hypokalemia.

What to do

Use continuous ECG monitoring perioperatively and check potassium and magnesium before anesthesia in patients on regular salbutamol. Correct electrolyte abnormalities and avoid giving extra high-dose beta-agonist around induction unless needed for bronchospasm.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
