Summary
Nortriptyline can enhance the cardiovascular effects of salbutamol, leading to tachycardia, arrhythmia or changes in blood pressure. Both drugs can contribute to QT prolongation and hypokalemia-related risk, mainly at higher beta-agonist doses.
What this interaction means in plain words
Ipratropium-salbutamol and Nortriptyline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ipratropium-salbutamol belongs to Respiratory; Nortriptyline belongs to Central Nervous System.
Why it happens
Tricyclic blockade of noradrenaline reuptake combined with beta-2 agonist cardiac stimulation produces additive adrenergic and arrhythmogenic effects.
What to do
Monitor pulse, blood pressure and ECG in patients with cardiac disease, and avoid frequent or high-dose salbutamol use. Check potassium if symptoms or high bronchodilator use occur.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
