Summary
Tadalafil is a vasodilator and can add to the blood pressure lowering effect of angiotensin receptor blockers such as irbesartan. Symptomatic hypotension, dizziness or syncope may occur, particularly after the first doses or dose increases.
What this interaction means in plain words
Irbesartan and Tadalafil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Irbesartan belongs to Cardiovascular; Tadalafil belongs to Genitourinary.
Why it happens
Additive vasodilation through PDE5 inhibition combined with angiotensin II receptor blockade.
What to do
Monitor blood pressure and orthostatic symptoms when starting or adjusting tadalafil. Counsel the patient to rise slowly and consider a lower tadalafil starting dose in patients with unstable blood pressure.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
