Summary
Tolmetin can reduce the blood pressure lowering effect of irbesartan. Combined use can also cause acute kidney injury and hyperkalemia, especially in elderly, dehydrated or renally impaired patients.
What this interaction means in plain words
Irbesartan and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Irbesartan belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.
Why it happens
NSAIDs block vasodilatory prostaglandins that maintain glomerular perfusion, while ARBs reduce efferent arteriolar tone, so renal filtration falls and potassium retention increases.
What to do
Monitor blood pressure, serum creatinine and potassium after starting and periodically thereafter. Maintain hydration and avoid the combination in patients with significant renal impairment or volume depletion.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
