Ketoconazole strongly inhibits CYP3A4 and markedly increases isavuconazole exposure. This raises the risk of isavuconazole toxicity such as hepatic effects and QT shortening.

What this interaction means in plain words

Isavuconazole and Ketoconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Isavuconazole belongs to Anti Fungals; Ketoconazole belongs to Anti Fungals.

Evidence level: High

Why it happens

Ketoconazole inhibits CYP3A4/5, reducing isavuconazole clearance.

What to do

Avoid co-administration; use a single azole. If unavoidable, monitor liver function and isavuconazole-related adverse effects.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
