Summary
Isavuconazole, the active moiety of isavuconazonium, inhibits CYP3A4 and P-glycoprotein and can raise quinine concentrations. Quinine may in turn modestly raise isavuconazole exposure through CYP3A4 effects. Higher quinine levels increase the risk of cinchonism and QT prolongation.
What this interaction means in plain words
Isavuconazonium and Quinine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Isavuconazonium belongs to Anti Fungals; Quinine belongs to Anti Parasitic.
Why it happens
Isavuconazole inhibits CYP3A4 and P-gp, reducing quinine clearance; quinine may weakly inhibit CYP3A4 metabolism of isavuconazole.
What to do
Monitor for quinine toxicity (tinnitus, visual disturbance, arrhythmia) and consider an ECG if therapy is prolonged. Reduce the quinine dose if adverse effects appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
