Summary

Volatile anesthetics sensitize the myocardium to beta-agonists, and isoproterenol is a strong chronotropic agent that can cause tachyarrhythmias. Ventricular arrhythmias and blood pressure swings may occur.

What this interaction means in plain words

Isoflurane and Isoproterenol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Isoflurane belongs to Central Nervous System; Isoproterenol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Myocardial sensitization by the volatile anesthetic to catecholamine-induced arrhythmia, plus additive beta-adrenergic effects.

What to do

Titrate isoproterenol to the lowest effective dose with continuous ECG and blood pressure monitoring. Have antiarrhythmic therapy available.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
