Summary
Itraconazole can raise vincristine exposure, increasing the risk of neurotoxicity such as peripheral neuropathy, constipation, paralytic ileus and seizures. Case reports describe severe neurotoxicity when vinca alkaloids are given with azole antifungals.
What this interaction means in plain words
Itraconazole and Vincristine liposomal can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Itraconazole inhibits CYP3A4 and P-glycoprotein, reducing vincristine metabolism and efflux.
What to do
Prefer a non-azole antifungal such as an echinocandin during vincristine therapy where possible. If the combination is unavoidable, monitor closely for neuropathy and bowel dysfunction and reduce or hold vincristine if toxicity develops.
What to ask your pharmacist
