Summary
Ivosidenib prolongs the QT interval, and donepezil can do the same, so the combination increases the risk of torsades de pointes. Ivosidenib is also a CYP3A4 inducer and may lower donepezil exposure to a degree.
What this interaction means in plain words
Ivosidenib and Memantine-donepezil can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ivosidenib belongs to Oncology; Memantine-donepezil belongs to Central Nervous System.
Why it happens
Additive QT prolongation; ivosidenib may also induce CYP3A4 metabolism of donepezil.
What to do
Avoid if an alternative exists. If used together, monitor ECG and electrolytes at baseline and regularly, and reduce or interrupt ivosidenib if QTc becomes markedly prolonged.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
