Summary
Ketoprofen can blunt the blood pressure lowering effect of trandolapril and may reduce kidney function, especially in older, volume-depleted or renally impaired patients. The risk of acute kidney injury and hyperkalemia is increased.
What this interaction means in plain words
Ketoprofen and Trandolapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ketoprofen belongs to Musculoskeletal; Trandolapril belongs to Cardiovascular.
Why it happens
NSAID inhibition of cyclooxygenase reduces renal vasodilatory prostaglandins and sodium excretion, opposing ACE inhibitor effects and lowering glomerular filtration.
What to do
Prefer a non-NSAID analgesic; if the NSAID is needed, ensure adequate hydration and monitor blood pressure, serum creatinine and potassium after starting and periodically.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
