Summary

Xipamide is a thiazide-like diuretic that lowers blood pressure, and combined with labetalol the effect is additive. This is often intended but can cause excessive hypotension, dizziness or falls, especially in older patients or those volume depleted. Diuretic-induced electrolyte disturbances may also matter.

What this interaction means in plain words

Labetalol and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive antihypertensive effects of beta/alpha blockade and diuretic-induced volume and sodium depletion.

What to do

Monitor blood pressure, orthostatic symptoms and electrolytes, and reduce doses if hypotension occurs. Start the combination at low doses in frail patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
