Summary
Lonafarnib is a sensitive CYP3A substrate, and lansoprazole may modestly raise its exposure. Lonafarnib also inhibits CYP2C19 and may raise lansoprazole levels, and the cardiac effects of lonafarnib are not fully characterised.
What this interaction means in plain words
Lansoprazole and Lonafarnib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Lansoprazole belongs to Gastrointestinal Tract; Lonafarnib belongs to Miscellaneous.
Why it happens
Weak CYP3A inhibition by lansoprazole raises lonafarnib levels, and lonafarnib inhibits CYP2C19, reducing lansoprazole clearance.
What to do
If coadministration is needed, keep lonafarnib at its current dose and monitor for palpitations, syncope and gastrointestinal toxicity. Adjust the PPI dose if adverse effects arise.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
