Summary
Lefamulin is a moderate CYP3A4 inhibitor and increases exposure to CYP3A substrates. Quetiapine is mainly cleared by CYP3A4 and can prolong the QT interval, and lefamulin itself prolongs QT, so the combination may add to arrhythmia risk. Lefamulin labeling advises against use of the oral form with QT-prolonging CYP3A substrates.
What this interaction means in plain words
Lefamulin and Quetiapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Lefamulin belongs to Anti Bacterial; Quetiapine belongs to Central Nervous System.
Why it happens
Inhibition of CYP3A4 raises quetiapine levels, combined with additive QT prolongation.
What to do
Avoid concurrent use where possible and choose an alternative antibiotic; if unavoidable, monitor ECG and electrolytes and consider reducing the quetiapine dose.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
