Summary
Lefamulin is a CYP3A4 inhibitor and can raise exposure to CYP3A4 substrates such as vilanterol. It also prolongs the QT interval, and increased beta-agonist exposure could add to cardiovascular effects like tachycardia and hypokalemia. The label contraindicates oral lefamulin with CYP3A substrates that prolong QT, but vilanterol is not a recognized QT-prolonging drug at inhaled doses.
What this interaction means in plain words
Lefamulin and Umeclidinium-vilanterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Lefamulin belongs to Anti Bacterial; Umeclidinium-vilanterol belongs to Respiratory.
Why it happens
Inhibition of CYP3A4 by lefamulin may increase systemic vilanterol exposure, with possible additive QT and adrenergic effects.
What to do
Monitor heart rate, ECG and potassium, particularly with oral lefamulin or other QT-prolonging drugs. Correct hypokalemia and watch for beta-agonist adverse effects.
What to ask your pharmacist
