Summary
Voclosporin, a calcineurin inhibitor, can cause hyperkalemia, hypertension and reduced glomerular filtration, and lisinopril also raises potassium and affects renal hemodynamics. Together they may increase hyperkalemia and renal function decline, although ACE inhibitors are often used with voclosporin in lupus nephritis for proteinuria.
What this interaction means in plain words
Lisinopril and Voclosporin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Lisinopril belongs to Cardiovascular; Voclosporin belongs to Miscellaneous.
Why it happens
Additive hyperkalemia from reduced aldosterone effect and calcineurin inhibitor-related tubular potassium retention, plus hemodynamic renal effects.
What to do
Monitor serum potassium and eGFR regularly, especially after initiation and dose changes, and reduce voclosporin per labeling if eGFR falls.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
