Summary

Combining tapentadol with the tricyclic antidepressant lofepramine can add to drowsiness and psychomotor impairment. Both drugs also have serotonergic activity, so serotonin syndrome and a lowered seizure threshold are possible, although lofepramine is less serotonergic than many other tricyclics.

What this interaction means in plain words

Lofepramine and Tapentadol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive CNS depression and additive serotonergic and noradrenergic effects, with possible seizure threshold lowering.

What to do

Start with the lowest effective tapentadol dose and monitor for sedation, agitation, tremor, hyperreflexia or other serotonin toxicity signs. Advise patients to avoid driving until the response is known.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
