Summary
Methylene blue inhibits monoamine oxidase A, and combining it with serotonergic or noradrenergic drugs such as tapentadol can cause serious serotonin syndrome. Tapentadol's labeling also warns against use with MAO inhibitors because of risk of serotonin syndrome and hypertensive crisis.
What this interaction means in plain words
Methylene blue and Tapentadol are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Reversible MAO-A inhibition by methylene blue combined with tapentadol's norepinephrine reuptake inhibition and serotonergic activity.
What to do
Avoid tapentadol in patients receiving methylene blue, and choose an alternative analgesic. If an emergency use of methylene blue is unavoidable, stop tapentadol and monitor closely for serotonin syndrome.
What to ask your pharmacist
