Summary
Lonafarnib is a strong CYP3A inhibitor and sensitive CYP3A substrate, and pimozide is a CYP3A4 substrate with a narrow therapeutic window. Raised pimozide exposure can cause QT prolongation and torsades de pointes.
What this interaction means in plain words
Lonafarnib and Pimozide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Lonafarnib belongs to Miscellaneous; Pimozide belongs to Central Nervous System.
Why it happens
Lonafarnib inhibits CYP3A4-mediated metabolism of pimozide, increasing its plasma concentrations and QT-prolonging risk.
What to do
Do not coadminister; the lonafarnib label advises avoiding sensitive CYP3A substrates with narrow therapeutic index such as pimozide. Use an alternative to pimozide.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
