Summary

Secobarbital, a barbiturate, induces CYP3A and may substantially lower lorlatinib exposure. Strong CYP3A inducers have caused hepatotoxicity with lorlatinib and may reduce its antitumor effect.

What this interaction means in plain words

Lorlatinib and Secobarbital are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Lorlatinib belongs to Oncology; Secobarbital belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Barbiturate induction of CYP3A increases lorlatinib clearance and reduces plasma concentrations.

What to do

Do not co-administer; use a non-inducing sedative. If an inducer must be stopped, wait about three half-lives before starting lorlatinib.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
