Summary
NSAIDs can blunt the antihypertensive effect of losartan and, when combined with an angiotensin receptor blocker, raise the risk of acute kidney injury, especially in older patients or those who are volume depleted. Potassium may also rise.
What this interaction means in plain words
Losartan and Meclofenamate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAID inhibition of renal prostaglandin synthesis causes sodium and fluid retention and reduces glomerular perfusion, which counteracts angiotensin receptor blockade and impairs renal function.
What to do
Avoid regular NSAID use where possible; if needed, ensure hydration and monitor blood pressure, serum creatinine and potassium after starting and periodically.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
