Summary

As with other NSAIDs, parecoxib can reduce the antihypertensive effect of losartan and, in combination with an ARB, increase the risk of acute kidney injury and hyperkalemia. Older or volume-depleted patients are at highest risk.

What this interaction means in plain words

Losartan and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of renal prostaglandin synthesis reducing renal perfusion, causing sodium retention and impairing potassium excretion.

What to do

Use the lowest effective parecoxib dose for the shortest time, keep the patient hydrated, and monitor blood pressure, creatinine and potassium.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
