Summary

Loxapine is itself an antipsychotic, so combining it with paliperidone duplicates dopamine blockade and sedation. This raises the risk of extrapyramidal symptoms, neuroleptic malignant syndrome and excessive drowsiness, and inhaled loxapine can also cause bronchospasm.

What this interaction means in plain words

Loxapine inhaled and Paliperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive dopamine D2 receptor antagonism and sedative effects of two antipsychotics.

What to do

Monitor for extrapyramidal symptoms, sedation and signs of NMS after each inhaled dose. Allow for the existing antipsychotic load when deciding on repeated loxapine dosing.

What to ask your pharmacist
