Summary
Ibutilide is a class III antiarrhythmic with a well-known risk of marked QT prolongation and torsades de pointes. Adding paliperidone, which also lengthens QTc, further increases this risk.
What this interaction means in plain words
Ibutilide and Paliperidone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Ibutilide belongs to Cardiovascular; Paliperidone belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic QT prolongation through potassium channel blockade.
What to do
Avoid using these drugs together. If ibutilide is required for cardioversion, consider holding paliperidone where clinically feasible, and use continuous ECG monitoring with electrolytes corrected.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
