Summary

Vandetanib markedly prolongs the QT interval and is labeled to be avoided with other QT-prolonging drugs. Adding paliperidone increases the risk of torsades de pointes and sudden death.

What this interaction means in plain words

Paliperidone and Vandetanib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Paliperidone belongs to Central Nervous System; Vandetanib belongs to Oncology.

Evidence level: High

Why it happens

Additive pharmacodynamic QT prolongation through cardiac potassium channel (hERG) blockade.

What to do

Avoid the combination and choose an alternative antipsychotic with minimal QT effect if possible. If unavoidable, monitor ECG, potassium, magnesium and calcium frequently.

What to ask your pharmacist
