Summary

Nirmatrelvir is a CYP3A4 inhibitor given with ritonavir, and the regimen markedly increases exposure to CYP3A substrates such as lurasidone. This may cause serious dose-related adverse effects including sedation and extrapyramidal symptoms. Lurasidone labeling contraindicates strong CYP3A4 inhibitors.

What this interaction means in plain words

Lurasidone and Nirmatrelvir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Lurasidone belongs to Central Nervous System; Nirmatrelvir belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4 by nirmatrelvir and ritonavir reduces lurasidone clearance.

What to do

Avoid during the treatment course; consider holding lurasidone or using another COVID-19 therapy, with specialist advice.
