Summary

Combining a serotonergic triptan with an antipsychotic has been associated in theory with serotonin syndrome, and altered dopamine blockade could add to neuroleptic malignant syndrome risk. Reports are rare and the risk with intranasal sumatriptan is low.

What this interaction means in plain words

Lurasidone and Sumatriptan intranasal have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Theoretical additive serotonergic effects (5-HT1B/1D agonism with 5-HT receptor modulation by lurasidone).

What to do

No routine change is needed, but monitor for agitation, tremor, hyperthermia, rigidity or autonomic instability after starting. Stop the combination if serotonin toxicity is suspected.
