Summary

Spironolactone is a potassium-sparing diuretic that causes little urinary magnesium loss and may modestly conserve magnesium. Any rise in serum magnesium is small and rarely matters except in patients with advanced renal impairment receiving large magnesium doses.

What this interaction means in plain words

Magnesium and Spironolactone have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Magnesium belongs to Gastrointestinal Tract; Spironolactone belongs to Cardiovascular.

Evidence level: Low

Why it happens

Reduced renal magnesium excretion through aldosterone antagonism in the distal nephron is proposed but poorly characterized.

What to do

No routine action is needed. Check serum magnesium in patients with renal impairment who are receiving high-dose magnesium together with spironolactone.
