Summary

Selegiline is an MAO-B inhibitor that becomes nonselective at higher doses and is labeled to be avoided with tricyclic and related antidepressants. Combined use has caused serotonin syndrome and severe blood pressure changes.

What this interaction means in plain words

Maprotiline and Selegiline are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Maprotiline belongs to Central Nervous System; Selegiline belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

MAO inhibition plus inhibition of noradrenaline reuptake increases synaptic monoamines, which can cause serotonin toxicity and hypertensive reactions.

What to do

Do not co-administer, and observe the washout period required in labeling when switching between the agents. If exposure occurs, stop both drugs and treat serotonin toxicity supportively.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
