Summary
MMRV vaccine contains four live attenuated viruses. Tacrolimus impairs T-cell control of these viruses, so disseminated vaccine-strain infection is possible and vaccine response is reduced.
What this interaction means in plain words
Measles mumps rubella and varicella vaccine live and Tacrolimus are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Calcineurin inhibitor-induced T-cell suppression allows uncontrolled replication of live attenuated viruses.
What to do
Avoid MMRV vaccine in patients on tacrolimus. Complete live vaccinations before starting immunosuppression where feasible.
