Summary

Upadacitinib suppresses immune function, so a live attenuated vaccine may cause uncontrolled vaccine-virus replication and disease. The vaccine response may also be blunted.

What this interaction means in plain words

Measles vaccine and Upadacitinib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

JAK inhibition impairs cellular and cytokine immune responses, permitting replication of live attenuated virus.

What to do

Do not give live vaccines during upadacitinib treatment. Complete live vaccination before starting therapy in line with current guidance, and defer live vaccines after stopping until immune recovery.
