Summary
NSAIDs may potentiate the hypoglycemic effect of sulfonylureas such as tolbutamide. Clinically significant events are uncommon with meloxicam, but sulfonylurea hypoglycemia can be prolonged and serious in older or renally impaired patients.
What this interaction means in plain words
Meloxicam and Tolbutamide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Meloxicam belongs to Musculoskeletal; Tolbutamide belongs to Endocrine.
Why it happens
Possible protein-binding displacement and altered sulfonylurea metabolism or clearance; the mechanism is not well established.
What to do
Monitor blood glucose when meloxicam is started or stopped and reduce the tolbutamide dose if hypoglycemia occurs. Advise patients on recognizing hypoglycemia.
