Summary

Methylene blue inhibits MAO-A and, with serotonergic drugs, has caused serotonin syndrome. Pentazocine has weak serotonergic liability, so the risk is lower than with other opioids but remains a concern. Reactions include agitation, hyperthermia, clonus and autonomic instability.

What this interaction means in plain words

Methylene blue and Pentazocine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Low

Why it happens

MAO-A inhibition by methylene blue increases synaptic serotonin, adding to serotonergic effects of the opioid.

What to do

Avoid pentazocine around methylene blue infusions; choose a non-serotonergic analgesic. If urgent methylene blue is needed, stop pentazocine and monitor for serotonin toxicity.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
