Summary
MDMA is a serotonin-releasing stimulant that raises heart rate, blood pressure and body temperature, while perphenazine can prolong the QT interval and lower the seizure threshold. Combined use may increase the risk of arrhythmia, hyperthermia and unpredictable CNS effects. The opposing sedative and stimulant effects may also mask intoxication.
What this interaction means in plain words
Methylenedioxymethamphetamine and Perphenazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive cardiovascular and QT effects with opposing CNS effects; perphenazine's CYP2D6 metabolism may also be inhibited by MDMA, which is itself a CYP2D6 substrate and inhibitor.
What to do
Advise patients to avoid illicit MDMA while taking perphenazine. If exposure occurs, monitor ECG, temperature and blood pressure.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
