Summary
Nalbuphine is a mixed kappa-agonist and mu-antagonist and may partly reverse morphine analgesia. In patients physically dependent on morphine it can precipitate withdrawal. In opioid-naive patients the two add to sedation and respiratory depression.
What this interaction means in plain words
Morphine and Nalbuphine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Morphine belongs to Musculoskeletal; Nalbuphine belongs to Musculoskeletal.
Why it happens
Nalbuphine competes at mu-opioid receptors as an antagonist or partial antagonist while adding to opioid CNS and respiratory depression.
What to do
Avoid giving nalbuphine to patients on regular morphine unless reversal of opioid effects is intended. If used, give small doses and monitor for withdrawal, loss of analgesia and respiratory depression.
