Summary

Procarbazine has MAO-inhibiting activity, so opioids may cause excessive CNS depression, hypotension or serotonin toxicity. Morphine labeling advises against use with MAOIs.

What this interaction means in plain words

Morphine and Procarbazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Morphine belongs to Musculoskeletal; Procarbazine belongs to Oncology.

Evidence level: Low

Why it happens

MAO inhibition by procarbazine may potentiate CNS depression and serotonergic or hypotensive effects of the opioid.

What to do

Avoid if possible; if morphine is needed, start at a reduced dose with slow titration and monitor sedation, respiration and blood pressure. Monitor for serotonin toxicity.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
