Summary
Procainamide is a class Ia antiarrhythmic that prolongs QT, and moxifloxacin adds to this effect. The combination increases the risk of torsades de pointes.
What this interaction means in plain words
Moxifloxacin and Procainamide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Additive blockade of cardiac potassium channels leading to QT prolongation.
What to do
Avoid concurrent use and choose an antibacterial without QT effects. If it cannot be avoided, monitor ECG continuously and keep electrolytes normal.
