Summary

Nefazodone has serotonergic activity that can impair platelet function, and when combined with an NSAID the risk of gastrointestinal bleeding is increased. The risk is greatest in older patients and those with prior ulcer disease.

What this interaction means in plain words

Nabumetone and Nefazodone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive impairment of platelet aggregation (serotonin uptake inhibition plus COX inhibition) together with NSAID-induced gastric mucosal injury.

What to do

Monitor for signs of gastrointestinal bleeding and consider gastroprotection with a proton pump inhibitor in high-risk patients. Use the lowest effective NSAID dose or consider an alternative analgesic.

What to ask your pharmacist
